The Pattern of the Syndrome of Inappropriate Antidiuretic-hormone Secretion Following Orthopaedic Surgery

Abstract
Purpose: To evalute the relerationship between urine output and serum antidiuretic hormone levels after orthopaedic surgery in order to establish the method of fluid therapy.

Materials and Methods: Ten patients who underwent spinal fusion, total hip replacement arthroplasty and bipolar hip hemiarthroplasty each for spinal stenosis, avascular necrosis of femoral head and femur neck fracture, were examined for the presence of the postoperative syndrome of inappropriate antidiuretic-hormone secretion. For each patient, levels of serum sodium and osmolarity, urine sodium and osmolarity, serum antidiuretic-hormone were obtained preoperatively and postoperatively.

Results: The investigation clearly showed that the syndrom occurred in each patient, with level of serum antidiuretic-hormone being highest post-operatively. This resulted in some reduction of urinary output, the reduction was maximum on the day of operation and gradually rising to normal over the next three days.

Conslusion: This syndrome and its associated reduced postoperative urinary output is common after orthopedic surgery and should be treated with restriction of fluid rather than administration of increased amount of fluid.
